The fixed dose combination (FDC) of Gliclazide (a sulfonylurea that exerts its hypoglycemic effect by stimulating insulin release from pancreatic ß-cells) and Metformin (an antihyperglycemic agent that reduces glucose absorption from the gastrointestinal tract and gluconeogenesis and enhances insulin sensitivity by increasing peripheral glucose uptake and utilisation) is used in the treatment for type 2 diabetes mellitus when either drug alone does not improve glycaemic control.

Precautions: Adjust dose of combination according to blood and urinary glucose levels during the first few months, however, there have been few reports of lactic acidosis in patients of renal or liver disease, safety and effectiveness in children have not been established.
Contraindications: Hypersensitivity, type 1 diabetes mellitus, severe renal & hepatic impairment, diabetic coma & diabetic ketoacidosis, cardiac failure, recent myocardial infarction, congestive heart failure, severe infection, acute or chronic metabolic acidosis with or without coma, stress trauma, severe impairment of thyroid function, dehydration, acute or chronic alcoholism, pregnancy & lactation.
Side Effects: Nausea, diarrhoea, gastric pain, constipation, vomiting, metallic taste in mouth, rash, pruritus, urticaria, erythema and flushing, headache and dizziness, gliclazide appears to be associated with a low incidence of hypoglycaemia, gliclazide may have the potential to produce adverse cardiovascular effects, impaired gastrointestinal absorption of vitamin B12 and folic acid has been associated with long term metformin therapy.
